Any work of this scope has had many helping hands. Originally, the chapter 5 material was conceived around published materials by Alan Smith (whose work I admire). As time went by, it was clear that his treatment had to be redone, but I have followed his design target miss data (DTMR data) in spirit. I have also had the opportunity to use unpublished material by George E. Rossmann and B. Rau. Again, it was my original intention to publish an edition much earlier. Their early contributions are now largely incorporated into several of the chapters in this book.
